I need to set up a page where a website visitor can leave their name, full mailing address, and questions or comments.  Then I need the page/website to either automatically forward that information to me (e-mail) or notify me that somebody left an inquiry.

I also want to add to my website some sort of mechanism (like the &quot;reCaptcha&quot; box below this comment) to prove that a real person made an inquiry or requested the online newsletter.

Do these items require customization by you, or am I supposed to be able to do that?

There was a change made to the login system a while back that affected clients on our oldest server.  I looked in our database and found that you&#039;re on our oldest server.  

Were you clicking on the icon on the &quot;contact us&quot; page to log into your site? Try logging in using the small &quot;Login&quot; link at the bottom of your website&#039;s home page (hhcocpas.com).  Click on that, and then use the administrative login credentials to get into the control panel.  If you need that information sent again I can send it to the email address we have on file, or someone can call to verify the credit card information we have on file and I can give it to you over the phone.
